Personal Trainer Request Form - Active Duty Only

Personal training is available at the MCAS and MCRD Fitness Centers. Each eligible adult patron can receive up to ten free sessions. Personal training is a free service for active duty members.
Sessions include but are not limited to the following:

  1. Consultation and completion of applications (Required)

1a. Medical release forms signed by your physician may be required as per ACSM standards. Sessions may not be offered until all required documentation is provided.

  1. Physical Fitness Assessment (Required)
  2. Balance Conditioning/Flexibility Conditioning
  3. Nutrition Consult
  4. Core Conditioning
  5. Strength Conditioning
  6. Strength Conditioning
  7. Strength Conditioning
  8. Strength Conditioning
  9. Cardiopulmonary Conditioning

All sessions must be completed within 60 days of Consult. Upon completion of all 10 sessions the personal trainers will provide each patron with their written exercise prescription.

You must be 18 years old and have a valid ID as an authorized patron of MCCS Fitness Centers.

All applications and assistance will be provided to you during your consultation.

If you have already received a personal training package, please bring it with you to your consultation.

Note: Frequent cancellations or no-shows for appointment may result in loss of sessions/services. Active Duty Military receive preference for services.
